New Application Form for the Grant or Renewal of a Local Practising Certificate
On 15 February 2012 a new version of LPB Form 1 Application for the Grant or Renewal of a Local Practising Certificate was approved for use by the Legal Practice Board (Board). The new form is LPB Form 1 version 5. This version replaces LPB Form 1 version 4 and the Annual Practising Certificate Explanatory Notes.
The result of this approval requires the Board’s online renewal portal to be taken offline temporarily for changes to be effected to its online form. This means that the Board will only accept the lodgement of hard copy LPB Form 1 applications. Please click on the link below to download the new form LPB Form 1 Version 5.
The Board will accept duly completed and signed LPB Form 1s if they are faxed to: (08) 9325 2743 or emailed to: general@lpbwa.com. However, applicants are to ensure that their original completed and signed LPB Form 1s, including any supporting documentation and payment, are received by the Board within 14 days of the date of your application.
